CAREPATH is a €5,267,50 Horizon 2020 project that proposes an ICT based solution for the optimization of clinical practice in the treatment and management of multimorbid patients with Mild Cognitive Impairment or Mild Dementia. CAREPATH solution will target a more complex multimorbidity scenario and follows an integrated patient-centered approach, in order to develop a flexible and modular system to deliver a best care, adapted framework for increasing the independence and Quality of Life (QoL) of multimorbid patients with dementia

ADLIFE is a European project funded by Horizon 2020 and involves 12 partners from 6 European countries, collaborating in research to improve the health and quality of life of the elderly population suffering from advanced chronic diseases. Our key areas of interest are digital health, innovation and integrated care. ADLIFE is going to demonstrate that it is feasible to provide a personalised integrated care to improve the health situation, deliver more appropriate targeted and timely care for patients with Advanced Chronic Diseases. The innovative ADLIFE toolbox will support early detection of care needs, patient empowerment and shared decision making and dynamic and personalised care provision.”

REDIM-CR

REDIM-CR is an NIHR CRN funded project that aims to design, develop and disseminate a novel regional standardised approach to the remote/virtual delivery of clinical research through consensus working with key stakeholders. To this aim, this project also targets hard to recruit groups which, alongside ensuring inter-operability and digital sustainability of the proposed improvement/innovation in remote/digital monitoring of clinical research, in order to facilitate rapid regional and national dissemination.

A COGNITANT RESPONSE TO HOSPITAL PRESSURES: PERSONALISING HOSPITAL DISCHARGE INFORMATION FOR UNDERSERVED POPULATIONS

There are intense NHS pressures to improve inpatient flow. This requires efficient discharge processes. Patients from deprived backgrounds are at greater risk at discharge, with increased readmission and mortality. Increased hospital pressures will exacerbate risks for deprived populations. These populations are less able to access post-discharge support. 

Cognitant are a global, award-winning company delivering bespoke locally co-produced health information. They have a delivery track record across NHS organisations, including underserved populations. With financial support from NHS Coventry and Warwickshire Integrated Care Board we will coproduce and implement supported discharge information for the 20% most deprived medical patients discharged from UHCW, reducing readmission and increasing post discharge support, enabled by robust digital inclusion activities.
